In NSW Australia, if the pharmacist mispacks incorrect medication for our client imto their webster packs, and we give the client their medication from that pack...if the client dies, we are considered the murders and are sent to prison. This is the way the law stands, so we go through the 8rights of medication each time,check and recheck and if in doubt,ring the office and advise. This can be a good thing in one way but the pharmacist should be help responsible as well. Mistakes can happen,but it it those mistakes that causes loss of life.

    That’s an oversight in the law, and a very important one, at that. I can see that you would be somewhat responsible in that case, but the pharmacist should still take his share of the blame!

      Exactly,hes the "expert’ and packing the websters. If clients had taken their own meds and if an autopsy was done they may pin the pharmacist,otherwise,write it off as " old age". Often,clients cant even read what is in their webster pack,let alone remember what all the tablets are for.
